Making Your Appointment
To schedule an appointment, please call the office location you would like to visit. If you have any questions, or if you should need to cancel your appointment, please contact us as far in advance as possible.
What To Bring
To make your appointment go as smoothly as possible, we ask that you bring the following items, if applicable:
- Insurance Card: For proper billing of your visit, we will need to record information about your medical insurance from your insurance card.
- Forms: You can download and print the New Patient Form on this website. You can fill the form out ahead of time and bring it with you to your appointment. If you prefer to fill the form out at the office, please arrive for your appointment 15 minutes early.
- Medication List: Please bring an up to date list of your current medications. Include both prescribed and over the counter medications.
- X-rays/MRI/CT: If you have had imaging studies taken that relate to your current problem, please bring them, or have your doctor send them to our office. This will save you the time and expense of having new images taken.
